The following resources are available for individuals participating in Kenosha Unified School District professional development.

Order Transcripts
KUSD participants who are looking to obtain a transcript can do one of the following:
ONLINE
Carthage works with Parchment to accept transcript orders online. Students can request official transcripts to be sent electronically or through the mail. Please click on the link below to enter your order.
BY MAIL
Download the Transcript Request Form. Please send this form along with the appropriate payment to Carthage College Registrar’s Office, 2001 Alford Park Drive, Kenosha, WI 53140. Checks should be made out to “Carthage College.” Please allow 5-7 business days for processing once we have received the request.
BY FAX
If a student has not yet received their complimentary transcript, a transcript request form can be sent by fax (262-551-6007). Download the Transcript Request Form
