Please fill out one form for each course. If the library owns a copy of the items requested, we will pull it from the shelves. If you have personal copies of items you’d like on reserve, please bring them to the Circulation Desk and we will have you fill out a paper form. If you are requesting more than five items for reserve, please use a second form. If you would like the library to order a copy of an item to be included in its permanent collection, please use this form.
